Tournament Order Form This is the form used for requesting the hosting of one of the TTA District or State tournaments. You may download this form and duplicate. All applicable fees must be submitted with the form. Your request for a District Tournament must be submitted by May 1st. Requests for State Tournaments for the following year must be submitted by seven days prior to the State Meeting date.
Scholarship Application TTA invites all affiliates to nominate candidates for the annual scholarship awards announced at our Annual State Affiliates' Meeting in August. Details are available on the form. Deadline for entry is August 1st.
Child Abuse Statement (Updated 2/10/06, ADOBE PDF) This is a three part form suitable for use in protection against child abuse in the Texas Teenage program. We highly recommend its use for all those who are associated with the youth. More details are available on the form.
The Indication Form is now located at First Base on the Home Page. This is our initial form that begins each TTA season. You may now simply fill out this form on-line and submit it to the State Office without faxing or mailing. This gives information on the league contact person TTA will correspond with for the year. Also, the number of leagues and teams planned for the season should be "indicated" in order that the state office knows the type of information needed. We ask that the Indication Forms be sent to the state office as soon as possible or by March 1st of each year. For any questions, contact the TTA State Office for assistance.
The Application Form is now located at Second Base on the Home Page. This is the Official form for applying for affiliation with TTA. One Application Form must be sent in for each League you are affiliating. Simply fill out your application form league by league and print each league application form. Send team fees with the forms to TTA at the address on the forms. Due Date is 30 days prior to the first official league game or no later than May 1st. The Team Roster form is now located at Third Base on the Home Page. This is the Official form for all teams and must be filed BY THE PLAYING OF THE FIRST OFFICIAL LEAGUE GAME of the season. (Adopted by the State Board of Directors October 2005). By using the on-line form, there will be no need to mail or fax the form once you have submitted the roster to TTA. You may and should print out a copy for yourself. Follow instructions by clicking 3rd base at the Home Page. For any assistance, contact the TTA State Office. The previous Tournament Roster is now replaced with a simply reporting form at Home Plate on the Home Page. You do not need to re-submit your origional team players. You only need to report your pickup players and certify that you are qualified as a tournament team. The TTA State Office plans to forward this certification along with your original Team Roster to the Tournament Director where you are assigned.
