Tournament Order Form This is the form used for requesting the hosting of one of the TTA District or State tournaments. You may download this form and duplicate. All applicable fees must be submitted with the form. Your request for a District Tournament must be submitted by May 1st. Requests for State Tournaments for the following year must be submitted August 1st.

Scholarship Application TTA invites all affiliates to nominate candidates for the annual scholarship awards announced at our Annual State Affiliates' Meeting in August. Details are available on the form. Deadline for entry is July 1st.

Child Abuse Statement (Updated 2/10/06, ADOBE PDF) This is a three part form suitable for use in protection against child abuse in the Texas Teenage program. We highly recommend its use for all those who are associated with the youth. More details are available on the form.
